• I certify that I have Professional Liability Insurance with Innersource listed as an Additional Insured Endorsement (AIE) on my policy. I will maintain my insurance for the duration of my referral listing on the Innersource website.
• I certify that I have Professional Liability Insurance and I am operating in a country outside the USA. I have tried to obtain an Additional Insured Endorsement (AIE) on my policy and have been unable to do so. I will maintain my insurance for the duration of my referral listing on the Innersource website.
• I certify that I am operating in a country outside the USA and have been unable to obtain Professional Liability Insurance. I have contacted Innersource and received approval for an insurance waiver.
